As a designated Social Care Network (SCN) agency, Nurturing Hearts provides help to
members with identified health-related social needs including nutrition, housing,
navigation and transportation. Medicaid members who are enrolled in a managed care
plan with complicated health conditions or members who are welcoming a new baby
may qualify for these services.
Nurturing Hearts provides fabulous, cooked nutritious meals (kosher and non-kosher),
grocery boxes with cooking ingredients, transportation, and care management — a
reassuring presence. Individuals can be eligible for services up to 6 months at a time
with an option of reauthorization. Nurturing Hearts is here to support members and help
them navigate the SCN services in the best way possible.
